The New Data Submission Form
A new standardized data Submission Form has been designed. It will help with a faster transfer of
information into the database.
Please submit missing information on a deceased member who is currently included in the database. Or, submit information on a
deceased member who has not yet been listed in the database.
Include as much data as the spaces allow in the Submission Form. Not every space has to be complete for transmission.
Sufficient space at the bottom of the Submission Form is available for 'Comments'.
